Depends on the country you are buying from.

In certain countries (US, UK, Australia, Germany, Japan, France, Italy, and Spain) you can buy Apple Care+ with Theft & Loss protection. Price will depend on the country.

In the US for instance, Apple Care+ with theft and loss coverage starts at $12.49 a month or $249 for 2 years.

In any other country you will need to contact an independent insurance company to insure the iPhone. Apple will only offer Apple Care+ for warranty extension and hardware service.
