How can I remove it from my phone I didn’t know who did this to me and he is accessing my information how can I remove it from my phone

Only you can set up Medical ID.
Did you purchase the phone used from someone? Is the information in the Medical ID for you or for someone else?
You can go to the Health app, tap your initials/icon in the upper right, tap Medical ID, tap Edit in the upper right, scroll to the very bottom and select Delete Medical ID
