Even if you are having AppleCare+ and damage is caused by anything but manufacturing defect you will need to pay a minimum charge to fix your screen. However that is still much lower than actual charges you might have to pay if you don’t have AppleCare+

Yes, unless the damage was caused by a manufacturing defect. Accidental damage incidents are cheaper if covered by AppleCare+, but not free.
