Does one need an iPhone to configure and use the ECG program in the Apple watch? Can it not be used as a stand alone product?

Yes, an iPhone is required to set up an Apple Watch. When set up, and paired the ECG program is enabled if available in a country which approves use of the ECG. If approved the Watch can use the ECG function independent of the iPhone, but details of the ECG will be limited compared to the information available on the Health App.
