You have a phone released 4 years ago. Batteries lose their ability to hold a full charge over time and the battery health on your phone is now degraded to the point it's time to have the battery replaced. Schedule an appointment at your Apple Store Genius Bar or Apple Authorized Service Provider and have the battery replaced for $69.
