I forgot the Apple ID and password that were used to set up my iPad

Use Apple's iForgot service or contact their Account Security team. If you're the iPad's original owner and you've encountered the activation lock, you can instead supply Apple with its purchase receipt and ask them to remove it, but this won't grant access to anything else associated with that Apple ID..
