Since my wife got her new Apple Watch incoming messages are not heard on her phone every time. She always gets a vibration on her watch but not always a ringtone on the phone. The mute button is not on

That is how notifications works. Since your wife has a Watch, then if the phone is locked, notifications are received on the Watch. Look in your list of Messages...does any Message have the MOON next to that Contact? Click on that Message, click on Details, and see if you have Do Not Disturb on for that specific Contact.
Receiving Messages but ringtone is heard only sometimes.
